Retinal toxicity from plaquenil years after discontinued

Chloroquine is now uncommonly used in favor of its derivative hydroxychloroquine. In the United States, hydroxychloroquine is most often used for its anti-inflammatory effects in rheumatology and dermatology.

Plaquenil induced retinal toxicity is reversible if detected early and plaquenil dosage altered or the medication discontinued. Even suspicious early plaquenil toxicity warrants communication with the patient’s treating physician, and meaningful coordination of care would include a report even if the findings are normal. A 71-year-old arthritic white woman requested a second opinion regarding retinal Plaquenil toxicity. The patients history was significant for seronegative rheumatoid arthritis diagnosed 6 years prior. An example of advanced toxicity is presented here. A 68-year-old female with rheumatoid arthritis was treated with hydroxychloroquine 200 mg PO bid for almost five years. She complained of bilateral visual disturbances and discontinued the medication. She reported progressive symptoms for over one year following discontinuation.

While early toxicity may be asymtomatic, patients with more advanced stage of toxicity may complain of color vision changes or paracentral scotomas. Its toxic effects on the retina are seen in the macula.

  3. Retinal Damage. According to Drugs.com, Plaquenil given for a long term can cause retinal disorders after a few months or years of daily use--especially if the dose exceeds 400mg per day. The risk of retinal damage increases at doses of 600mg to 700mg.

PLAQUENIL hydroxychloroquine sulfate tablets is indicated for the treatment of rheumatoid arthritis, and discoid and systemic lupus erythematosus, in patients who have not responded satisfactorily to drugs with less potential for serious side effects. After seeing MANY professionals including a neurologist, a retinal specialist, neuro-ophthalmologist, and undergoing MANY different tests, she was diagnosed with Plaquenil toxicity. She does have field of vision loss, mostly on things that are close up. She took Plaquenil for almost five years before having any symptoms. Another speculation, based on the macular localization of the toxicity, is that light absorption or cone metabolism may play a role in its effect. 1 Clearance of the drug from blood and urine can take months to years after discontinuation, and a few patients have shown continued toxicity years after hydroxychloroquine treatment was stopped. 3

Plaquenil and Hashimoto's - HealthBoards I was seeing a rheumatologist who did every test possible from lupus to MS and rare diseases in between. No firm diagnosis. At one point, my hair was falling out and I was feeling very tired and brain-fogged so the rheumatologist prescribed plaquenil which is a drug used in malaria, for rheumatoid arthritis and lupus.
