Malaria: Treatment of uncomplicated malaria caused by susceptible strains of Plasmodium vivax, Plasmodium malariae, Plasmodium ovale, and Plasmodium falciparum; prophylaxis of malaria in geographic areas where chloroquine resistance is not reported. Limitations of use: Hydroxychloroquine is not effective against chloroquine- or hydroxychloroquine-resistant malaria strains of Plasmodium species; not recommended for treatment of complicated malaria, malaria prophylaxis in regions with chloroquine resistance, or treatment when the Plasmodium species has not been identified; hydroxychloroquine does not prevent relapses of P. ovale infections because it is not effective against the hypnozoite forms of these parasites.

Lichen Planopilaris LPP is a form of scarring alopecia and is a rare inflammatory condition that results in patchy permanent hair loss on the crown and vertex. It destroys the hair follicle and replaces it with scarring. Symptoms can include an intensely itchy scalp and scalp tenderness. LPP is considered a follicular form of Lichen Planus. Plaquenil has been used because of its benefit in patients with discoid lupus erythematosus, another lymphocyte-mediated scarring alopecia. Isotretinoin, dapsone, and thalidomide have all been tried. The biologics are also being tried for LPP. In my experience, LPP waxes and wanes over time, probably without regard to treatment. Published evidence says that it is mediated by T-lymphocytes. Plaquenil is an anti-inflammatory drug approved by the FDA for malaria, lupus erythematosus, and rheumatoid arthritis. It has an effect on T-lymphocyte mediated inflammation, making it a logical choice for a treatment trail for alopecia areata.
